Application of Business Permit

Who must avail of the service?

All constituents who plan to operate a new business or to continue operating an existing business are required to secure Mayor‘s Permit.

APPLYING FOR WALK-IN BUSINESS REGISTRATION

Requirements for New Business

  • Unified Application Form
  • Cedula
  • Fire Safety Inspection Certificate (BFP)
  • Occupancy Permit (Building Official)
  • Zoning Clearance (Planning Office)
  • Environmental Compliance Certificate – if Resort
  • Sanitary Permit (Health Office)
  • DTI Business Name Registration (Original & Photocopy)
  • SEC Registration for Corporation (Original & Photocopy)
  • CDA Certification for Cooperative (Original & Photocopy)

Requirements for Renewal of Business Permits

  • Cedula
  • Fire Safety Inspection Certificate (BFP)
  • Occupancy Permit (Building Official)
  • Environmental Compliance Certificate – if Resort
  • Sanitary Permit (Health Office)
  • DTI Business Name Registration (Original & Photocopy)
  • Audited Financial Statements(“AFS”) or unaudited AFS for those who are not required to file AFS with BIR
  • Sworn Declaration of Gross sales or Receipts
  • Income Tax Returns
